WAIVER AND DISCHARGE RELEASE

This is a release of liability by the person who has signed and printed their name on the bottom of this form of their own free will (here in after referred to as the “participant”/”participants” or is the guardian of said participant(s)). Said participant(s) are being permitted to participate in an Ambush camp hosted by the St. Louis Ambush/Ambush Soccer Club LLC (the "activity"). I as representing myself or representing any group of minors (if applicable), and any respective legal guardians, assigns, heirs, and next of kin, agree and represent that I understand the inherently dangerous nature of the activity and that the participant(s) is/are qualified, in good health, and in proper physical condition to participate in such activity. I further agree and represent that if at anytime I believe the activity to be unsafe for the participant, for any reason, that participant(s) will immediately discontinue further participation of the activity. I am agreeing to release Academy of the Sacred Heart (St. Charles), the St. Louis Ambush, Ambush Soccer Club LLC, and any of their subsidiaries from liability. I have therefore been advised to read this document carefully. I am over the age of 18 years, or represent a minor (or minors). I FULLY UNDERSTAND THAT THE ACTIVITIES ARE INHERENTLY DANGEROUS AND INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, POTENTIAL EXPOSURE TO COVID-19 AND OTHER INFECTIOUS DISEASES, WHICH COULD RESULT INPERMANENT DISABILITY, PARALYSIS, AND DEATH (“Risks”); and these Risks and dangers may be caused by the Participant’s action or inaction, the action or inaction of others participating in the Activity, the condition of the premises in which the Activity takes place, and there may be other risks either not known to me or not readily foreseeable at this time. I recognize that the Indemnified Parties (as defined below) are not obligated to permit the named participant to participate in or observe the Activity. In consideration of my being permitted to participate in the Activity, I HEREBY AGREE TO ASSUME SUCH HAZARDS AND RISKS and release the Indemnified Parties as set forth below. I further agree to discharge, waive, release and covenant not to sue Academy of the Sacred Heart (St. Charles), The St. Louis Ambush, Ambush Soccer Club LLC, and each party's respective subsidiaries, partners, members, managers, officers, directors, employees, agents and affiliates (collectively, the “Indemnified Parties”), from all claims, demands and liabilities for any and all property damage, personal injury and/or death arising from my participation in the Activity. This Discharge, Waiver and Release includes all claims, demands, and liabilities, whether known or unknown, foreseen or unforeseen, future or contingent, and includes claims, demands and liabilities arising out of the negligence of the parties so released. Furthermore, if permitted by applicable law, this release also includes claims, demands and liabilities arising out of the gross negligence or willful and wanton negligence of the parties so released. This Discharge, Waiver and Release also include claims, demands, and liabilities for indemnities and contributions arising from property damage, personal injury and/or death to a third party.

I HAVE READ THIS FORM AND FULLY UNDERSTAND ITS TERMS. I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL LEGAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY, AND I INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY OF THE ACTIVITY PARTIES AND INDEMNITY PARTIES NAMED IN THE WAIVER TO THE GREATEST EXTENT ALLOWED BY LAW. I AGREE THAT IF ANY PORTION OF THIS WAIVER IS HELD INVALID, THE BALANCE SHALL CONTINUE IN FULL FORCE AND EFFECT.

PROGRAM OVERVIEW

FC Ambush

Staff

Technical Director – Corey Adamson, Corey@stlambush.com

Director of Operations – Donnie Alberty, Coach@stlambush.com

Players Development

From the age of U6 through U12, while playing in 4v4, 7v7 and 9v9 games, our main focus will be technical training, and critical thinking. We will emphasize the use of proper technique and fundamental soccer skills (fast footwork, ball possession, first touch, ball distributions, etc.). We will teach our players to be comfortable handling the ball in tight spaces, comfortable to take on opponents in 1-on-1 situations, and, most importantly, how to create and find space with or without the ball.

We will provide players with a lot of opportunities to consistently compete in an environment that fosters skill,

creativity, mobility, possession, and confidence.

Code of Conduct

Coaches are ultimately responsible for the players and parents behavior during games. We are collectively representing the Club and we should be mindful of our behavior at all times. The guidelines are posted on GotSport website which you have to agreed on at the time of registration.

Ø      It is extremely important that instructions are given by the coaching sideline only, and not from the parents’ sideline. This helps the coaches to send consistent messages to the players and avoid confusion.

Ø      Please limit sideline cheering to positive encouragement and kudos only. Any comments which can be perceived as “negative” by the players can easily affect their confidence level. Please be your player’s biggest fan.

NOTE: Please see SLYSA website https://www.slysa.org/ for your reference.

Note: This is a competitive league and participation in the selected tournaments is highly encouraged as the absence of a player affects the whole team. At a minimum, we ask that players join the two tournaments included in the fees. Please know that the team’s performance in the SLYSA league and tournaments impact the position of the team in the rankings.

#NoPlaceforHate

Bullying is not tolerated at all in our program. The players are now at the age where parents and coaches will need to pay extra-close attention to how our players are interacting with each other. Bullying can be in a form of physical and/or verbal abuse. With easy access to electronics, bullying can also be delivered in a form of texting and/or any social media posting (Facebook, Instagram, etc).

Please refer to MYSA’a guidelines in which respect to suing slurs, gestures against their opponents and other around the game. The Laws of the Game includes penalties for this type of behaviors. For more please refer to MYSA website (https://www.missourisoccer.org/news_article/show/1159888).

Playing Time

Please note that in Select soccer, there is no such thing as equal playing time. The coaches, at their discretion, will be using six factors in determining playing time for each player based on:

Ø Player’s competitive drive, desire and hustle during game/practice/training

Ø Player’s contribution to their own development and desire to learn and improve

Ø Player’s contribution to team development
